Please check the practice boundaries in the below to see if you can register with us. Our Practice boundary area is Exmouth, Lympstone, Woodbury, Budleigh Salterton, East Budleigh and Exton.

New Patient Registration
The Doctors welcome new patients who live within our practice area. If you are new to the area and wish to register with our Practice, please speak to one of our receptionists who will assist you and give you the relevant documentation or you can download the patient registration form below, email it to us (please bring photo ID to your first appointment), drop it into reception or pop it in the post. If you have a preference for a particular GP, please mention this when you register and we will try and accommodate your wishes.
Medical treatment is available from the date of registration. Please contact us for further information.
New Patient Checks
All new patients, who register with our practice, will be offered a health check which will enable us to obtain details of your medical history and provide other advice, as appropriate. Please remember to bring a sample of urine with you, when you attend for your appointment.
You may print off both of the registration forms below, fill them out and bring it in with you along with some photo identification. Please bring these forms along with you when you first come and visit us. Please make sure you complete both of the forms to complete your registration with us.
PLEASE NOTE: In order for you to register with us, you must either complete the Online NHS Registration OR the New Patient Registration Form (both are below).

National Data Opt Out
The national data opt-out was introduced on 25 May 2018, enabling patients to opt out from the use of their data for research or planning purposes, in line with the recommendations of the National Data Guardian in her Review of Data Security, Consent and Opt-Outs.
Patients can view or change their national data opt-out choice at any time by using the online service at www.nhs.uk/your-nhs-data-matters or by clicking on "Your Health" in the NHS App, and selecting "Choose if data from your health records is shared for research and planning".
